Feedback Form

The Feedback Form page allows you to provide feedback for subsequent review and follow up by content managers.

Lodging feedback for a particular Site

To lodge feedback for a particular Site:

• Select Click here, if you believe the information is incorrect from the Site Details page to display the Feedback Form with the name of the Site displayed
• Enter the feedback details. 

Lodging feedback that may not relate to a specific site

To lodge general feedback:

• Select from the navigation menu to open the Feedback Form
• Enter the feedback details. 

Entering Feedback details

Feedback Field Descriptions
Field	What to do
Site Name	This field is filled in by default with the name of the site that you are submitting the feedback for. You cannot change this.
Your Name	This field is filled in by default with the name associated with your login.
Your contact e-mail	This field is filled in by default with the email address associated with your login. This is a mandatory field.
Your contact number	Type your contact phone number.
Subject	Type a one line subject description. This is a mandatory field.
Comments	Type the details of the feedback or change that you require to be made to the service. This is a mandatory field.

After adding the Feedback details, select from the following:

To ...	Then ...
Submit feedback	Click .
Exit without saving Feedback details	Click .
