Agency An Agency is an entity that operates one or more Sites.  An Agency can have the same name as the Site. An Agency can also be a sole practitioner.
Site Site is the physical location or an address where the Agency is based.
Service Service is a single or multiple list of services offered at a Site.
Practitioner A user of the HSD database and data contained therein, for the purposes of supporting the delivery of health and related services to consumers (including the process of referring consumers to other services/practitioners)

Changes made to data for your agency and sites need to be approved, by someone with Agency Approver authority, before being installed and becoming available in searches.

  1. An Agency can have more than one site.
  2. Agency name can be the same as the site name.
  3. Several services can be associated with a site.
  4. More than one practitioner can be associated with a site.
  5. Any particular service can have more than one practitioner associated with it.
